Write a simple iPhone application

I need a simple application for testing. It has 3 pages,
1) first page few notes and agreement button
2) 40 buttons on the page to select which test they want to take
3) by selecting each buttons a pop up should ask how many sec they want to get for that particular test, there should be a check mark inside that pop up where they can disable that question on each and every question...
4) the last page(page 3) a report will be shown, says you have selected (this many questions) and you have this much time in total
5)also 4 group of questions to be shown and says these questions have this much percentage from total score.
6) Owner has different group of team to add formula for choosing those questions.
a place holder needed with dummy formula (there are two formula in the system for it)
7) This app to be used only 3 times, it can be used more than 3 times but another formula will be applied to the result.
8) if it shared with social media another 3 times used will be added. After a week again another 3 times can be added if it is again shared....
Detail spec will be send only to short list people. this is a quick project only 3 pages.... and very simple app, not much budget considered for this project.
