automatic reminders to renew password accounts

I have a database-driven website with members who have access to a protected area of a website by means of a password and login. Passwords are generated by purchasing an account in PayPal and automatically sent out right after purchase. Passwords are valid for one year. The login name is always the email address. When the passwords for a specific user is 5 days away from expiring, I need to have a code which will automatically send out a reminder email for them to renew their membership. See the attached file to get an idea of the database. It is an ASP page. The database was generated in MySQL. It is a windows server.
Basically, I need to setup a scheduled job - a piece of code that would check all the expiry dates and send reminders when they are 5 days out, and then the scheduler function of the operating system would run that code one time per day, completely automatically.
