Upgrade Wordpress Comments - Threading

I'd like to update our theme's [url removed, login to view] file to take account of Wordpress's new core functions, plus an extra feature.

I'd like to add:

* new simpler comments loop

* threading (i.e. reply to comment) with javascript

* password protection check

* odd, even, alt, author - css hooks

* backwards compatibility (see: [url removed, login to view])

Outside of Wordpress's core function, we would like to add a:

* Simple text editor as part of comment field (with bold/italic/underline, link/unlink, sub/superscript, indent/outdent, but without font size/style/alignment.

You will not need to do any CSS styling, but you will need to ensure the current classes & IDs stay the same.
