Changing the design of a single Wordpress php Page

I need a good DESIGNER.
I need to change the look and feel of a single page. The page is part of a Wordpress theme.
Please take a look at the images attached.
The code within the .php file should not be changed too much, otherwise the other stuff within the page would not work (example: mapping of the uploaded images)

Description (please take a look at the files attached to understand the description):

# Step1 - upload your photo:
- please design the upload form, as you see my design in the image.
- please use or change the upload form to be something like ajax or so. No page refresh or loading other page. After uploading the image it should be displayed directly at the Step3 (see Stage3)
# Step2 - select Time and Location:
- for the time picker, please use the following link ([url removed, login to view]) i need exactly the same time picker. You can download it from the page, its free.
- as for the location picker, please use the same code, but just adapt/design the input form to look like the time picker.
# Step3 - crop your photo and save:
- after uploading the photo at the stage1, the image should be displayed here to be able to crop it.
- The cropping must be 600x800px, to keep the image ratio as other images. Just to keep the same size for all the images.
- You can use here any Wordpress cropping tool you want.
