Custom coding for wordpress gravity forms project

I need some custom coding developed for a wordpress membership site I am developing. I need a Prospecting Tracker developed. I have looked at Gravity forms and viewer for this but need some customisation. Users will track targeted vs actual tasks for 5 areas and when they complete the tracker, I need the results to show up visually in a bar or pie chart and trigger an email to themselves and their manager. I need this done in 3 days.
I have added a mock up of the page where the tracker will sit and the 5 elements - you can see target vs actual. They will update this Monday - target for the week Friday actuals for the week. I will need to be able to store the data, so that I can report back on progress of my participants on a monthly basis.
