Shopify store wanted - Custom design and coding elements

We require a website to be constructed on the Shopify platform.
A ready made template would be preferred but you can do this custom if you wish.
Some off the shelf modules will need installing too. There maybe requirement for some custom development but hopefully not as we are on a budget for this project and also we would like it completed as fast as possible.
This is why we have made every effort to document all requirements in the documentation attached.
If you feel you want to go ahead with this project then we would require the following:
1. 3 examples of recent work completed on a Shopify website
2. 3 people we can contact for a reference on your work
3. A reason why you think you would be a good first for this project
4. Please read our documentation attached and advise us if you think that Shopify is the best platform to use. If you don't think so then let us know what you would consider and why. We will consider what you say.
On-going development and tweaks will be required so if you do good work then we would like to continue using you. See our recent employer feedback. We gave a designer around 5 jobs in quick succession when they delivered good work for us.
Any questions feel free to ask. I want this project done within 2 weeks if possible so we will reward the job quickly if we find the right freelancer.
