Hi,
I wanted to create a survey on my own website. The details are:
1) Questions with multiple options and possibility of selecting more than one option as also an "Other" option where the user can enter any text
2) We need to create a backend database for the survey where we can calculate the total for any question or know how many user selected option 1 for question 1 and so on
3) Export all the question and answers to Excel in a clearly readable format. Responses which are in numerics should be transferred to Excel in numeric form
4) User should be allowed to enter his address- country , state, zip
5) The user will be clicking on a link on a desktop application when he reaches the survey page and would already have registered his email address. The survey should pick up the user's email address based on the link ( we can discuss this further)
6) We should have an option to change the questions and options and increase /decrease the questions
7) Prompt the user whenever he leaves a required field blank
8) User should be able to select the options through both mouse and cursor
9) Have a link from this page to another page which has text and graphics (text and graphics will be provided by me)
10) Custom design for the survey and logo of the company (will be provided by me) needs to be embedded on the survey page
11) Like button with corresponding link should be there
