Kurzbeschreibung des Wettbewerbs

We need a one page form designed. Use attached jpg layout and text. You can change the color and styling of the font and layout as needed. we don't have a logo yet, so just use that area as a placeholder. Design should focus on helping people fill out the form, it should be intuitive, the experience should be one of holding the prospect by the hand.
