Kurzbeschreibung des Wettbewerbs

Hi there,
I need a very simple app designed ideally in Figma although other design tools are allowed.
I have very short timeframe (about a week) to finish the design.
The app will have 5 screens:
1) Login screen
2) Select mode screen (mode will be the type of the form)
3) 3 forms with multiple form fields essentially replacing paper forms. One of the forms attached

The requirements are:
1) Clean an intuitive and very simple design
2) High contrast - the app will be used outdoors 100% of the time
3) High accessibility - the app will be used by older generation

The app look has to work on both phone and tablet although in 90% of cases it will be used on a tablet.
