I have more than 5 yrs of exp in iOS Programming. This requirement is fairly simple. All you need is to provide a web service to me and let me know what fields you require to be filled in the form by user and the app will send these fields to your web-service. I also have gained android experience over past years and I have another android developer with me to work on your project. Working with an individual with a small team would be beneficial (cost wise) for you rather than going for a company.
