Create a form using Gravity Forms for WordPress

I need to create a form which is made up of a couple of options on each slide, these will buttons.
When an option is selected the form must bring up the next options like an image slider.
The backgrounds for each form slide may change.
The website must remain on the home page when the form is in use and when the form is submitted.
Images will be provided or placeholders can be used and I will switch them out.
I have the website installed on WordPress and I am using the Gravity Forms plugin, we can use another solution if you recommend it.
Looking to have this project started and completed as soon as possible.
