Hi, I can do your project , first I will convert your form in html5, when press submit then javascript will create json or csv data(you will easy choose between json and csv with javascript variable) and send to php script (php is server language and must use to send email, I will use ajax request when press submit that is better because not refresh page), that html form I can save in pdf with mPDF library if you want. Hi, I have checked your document and I can create a webpage to take the survey. On form submission, csv or json file can be created. However the file cannot be send to email purely using javascript. To send the file to email ID, backend coding will be required. Based on environment where you are hosting, backend coding can be chosen as php,node.js, etc. I can create a PDF file also using javascript. But as i send to mail it, backend coding will be required. Otherwise users can download the file and mail it manually, which might put off some users. Pls PM me to discuss further details.
