Database set-up / Implementation

Looking for someone to create a database driven online directory for "Life Coaches" integrated into a new site design. PHP / MYSQL is preferred. Here's the specifics: The existing site is here: <[login to view URL]> The new site mock up is here: <[login to view URL]> The database requirements need to be implemented into this existing site. I need four things that involve programming: 1. a user profile application form with the exact same options, etc. as this: <[login to view URL]> Their email will be their username, they make up their password. When the user hits "next" - their first/last name need to be checked against an existing database with an existing list of first and last names. IF their name is on the list, the other information from their form is added to the database. If it is not on the list, they are denied. 2. a control panel where registered users can log in to edit their existing user info (from the form) - plus add a paragraph about themselves, as well as upload a photo that automatically optimizes and sizes for viewing on the site. 3. A search function that uses name (first and/or last), country and/or city and/or state, ans well as any terms in their bio paragraphy as search options. The form for this function needs to be on this page: <[login to view URL]> 4. A system that would organize and display data when called upon. When you click your country on the world map (<[login to view URL]>) then your region - it lists all of the profile info for the members in that region. Like for the western U.S. here: <[login to view URL]> I'm sure there will be some questions and I am open to options /suggestions as to how the whole process works, etc. Thank you. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform cross platform / cross browser
