Create Schedule Appointment Form to work in DNN website

Pls study attached screenshots. Page 1 shows a simple form with drop down list and a button (which is initially greyed out). The link on the button depends on what the user selected in the drop down list. If the user selects "I will use another insurance" it directs to Page 2 All other selection will open a link in another window. Page 2 is self-explanatory.. the data will be sent to an email address.. Pls let me know if you have questions. It will be easy for me to select the winner if you can show me a sample of Page 1.. especially the button functionality.
