I would like to have a web form for a conference registration. The form will have:
1) Text fields where people will insert some data and check some of the options according to the attached form (actuall dates, amount will be available tomorrow)
2) The total amount due according to user selections on the web form ,
3) There will be a "PRINT" button which will print an invoice according to the options checked.
4) There will be a "SUBMIT" button which will e-mail an invoice according to the options checked (to a predefined e-mail).
Invoice format can be anything printable (PDF).
The page will be linked to another web page. It does not need to have a back end - database. It can be on the fly. The users that would like to register will fill the form and either
a) Submit it (automatically send through e-mail) or
b) Print and fax it manually.
