Simple Flash Template Customization

This project involves taking a flash template from template monster( [login to view URL]) and customizing it for a Chiropractic/Massage clinic. Some of the customizations include. 1. Simple logo design (the logo already exists, it is just the letters WFC. All you need to do is create a digital version to use on the site) 2. Instead of the fancy transition screen that is used when changing pages in the site, I need that removed and replaced with a simple and quick fade through. 3. Keep the top 1/3 of the website that contains the top banner and the navigation menu the way it is. The bottom portion of the website will contain text and a few images. I will prepare the content for you. 4. A 'New Client' form that visitors can complete before their first visit to the clinic. The form already exists in paper form, but an online version is needed. The form is 2 pages long with standard medical type questions. There is no need to create a database to store the information. All that is required is that when the form has been completed, it be emailed to the clinic in a format that will allow them to edit/print it. I do not need hosting for this site. Happy Bidding.
