This project involves creating 5 very straightforward web forms (if you follow the directions as they are clearly outlined) for my website. We currently have several forms on our [login to view URL] website but I want to make them look more professional and user friendly by alerting them when a box is missing and by sending them an email afterwards. The five forms and how I want them to be done are listed in the deliverables section. I am doing a full makeover of my current site and everything has changed so I will send you website files for the new site which is obviously missing a lot of the information as I am slowly working on finishing it all in a timely fashion. You can see the new page layout design by visiting this link here [login to view URL] General Rules: - I want all forms to be very well laid out, for an example of what I mean just visit any major business website [login to view URL]://[login to view URL] However you decide exactly to do it, just make it consistent across all forms and very professional looking. - For each section on the form I would like it to be laid out just like this [login to view URL] (I don't know how to explain it without sounding confusing)...so basically have the box around every section *I make it clear on each form when a new section starts. -Please use CSS to properly format the forms. Use the Times New Roman font (I use Times New Roman, Times, Serif for word formating) - I will give all the files to the winning bidder as soon as I accept them. You have lot's of time to do the project as I still need to update the rest of the website while I get this done. - Please make the forms very easy to read and understand when they are sent back to me. - This is specific to the 'email' field but it should be validated as an email address. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. ***** Project Description: *Create 5 web forms for my customers to fill out as listed below which include validation and a confirmation email. ***** Form #1 -- [login to view URL] Title '[login to view URL] Contact Form' Section Title 'Your Information' *First name / *Last name *Email Company Position Phone Number *Comments (have a big box below which allows them the option of typing in whatever else) Submit / Reset ---For all the fields that are marked with an asterisk they are required fields and a text box should pop up when they try to submit the application saying which fields need to be filled in...also if the required fields are not filled in the text should be changed to red (same for all five) --Send them this email Thank you [insert first name and last name here] for contacting us, We are working very hard to answer your question and (or) comments.
