Job Name Creator

Please put correct pricing, I need a software that will convert data entered and create a Job Name No login needed, I will be added the code to a existing database Data Collected: (First Name*, Last Name*, Company Name, Job Type* [drop Down box], Development Name, Job address*( [street, city, zip], and Suite number/letter *required fields The job name will be created based on the information entered A number will be generated first based on job type based on what type was added first (1 Commercial, 2 Residential) then an unique 3 digit will be created first based on company name (All jobs for that customer will start with the same 3 digits) If no company name, it will be based on first/last name first 4 letters of development name (if left empty don't add anything) street numbers first 4 digits of street name Suite number/letter I added an example of what was filled out and the Job Name that was created If Company name or name of person changes the number will still be the same The company/Name number will not change and will be the unique ID
