Customized Medical Quiz Script needed, Wordpress or similar is an option

What we basically need is a questionnaire. By the questionnaire you should be able to determine the perfect medication for the individual patient. This includes around 10 different Medicines, each with a different profile (e.g. in terms of strengh, long/short acting, side-effects, if can be taken with/without food etc. etc.). The required information on the individual patient should be provided only via check boxes, drop down options, rating scales (e.g. not important to important), not via text. Based on the input, specific meds will be excluded entirely, will get a higher rank (because it fulfills patient's needs) or a lower rank (because doesn't fulfill patient's needs so well as others in the choiceline). On the results page it should show the possible meds that are suggested based on the given input. Maybe with a picture. It is very important that the developer provides information in what way he needs the information. I'm aware that this might strongly depend on what software is used (you are free to use customizable out of the box solutions if that suits well). But it is really important that you tell us how exactly you are going to do it, because we must adapt our data to it. Requirements/skills: You should definitely know what you are doing and you should have done something like this before. At this time we don't require a specific design, just a working script, but it can't hurt if you know a little bit of how to make things look good.
