Build a Small 3 or 4 Page Website

I need a website built for my business. I have provided most of the content in the project files, however, I will be finishing the content while you start designing the website. LENGTH: Maybe 1 page...or maybe 3 to 4 pages. There won't be a lot of content so it's up to you if you only want to make it one page or different pages for each section. FORM: I also need a form on the homepage with a conditional decision field that only appears if the check boxes in the field above it are selected. However, I have a subscription to Zoho Forms and have already created the form so the code for the form can be pasted into the homepage. So I only need you to design the form if you can make it better than the Zoho form. DEADLINE: I need the entire website completed 48 hours from the time this project is submitted. I have flyers being delivered to houses in two days which advertises our website so it needs to be completed by then. INSTRUCTIONS: 1. **Read the "1. Website Notes" file.** PLEASE READ THIS ENTIRE PAGE! I want the designer to understand my goals and what I'm using the website for so we are on the same page. Too many times I provide files like this on Freelancer and then it's obvious the designer never looked at the files I've provided which is very frustrating and causes me to have a bad experience with that designer. 2. Open and look at the "2. Info Request Form" file 3. Look at both the "3. Current Flyer - Front" and "4. Current Flyer - Back" file. 4. Ask me any questions you may have. IMPORTANT: It's very important that this website look very professional, clean and organized. I want it to give off the impression of a successful business.
