I need a jQuery form created. Show all the questions one at a time. After the first question, also show how many are remaining and include a back button. (need radio button name=num_mtgs, three options= One, Two, I Don't Know) Question 6: What is the estimated value of the home today? (need text field value) Question 7: How many bedrooms (need dropdown name num_br, values are 1,2,3,4,5+) Question 8: How many bathrooms (need dropdown name num_bath, values are 1,2,3,4,5+) Questions 7 and 8 can be on the same page or show together. Question 9: Please give us all additional information here (need textfield, name=notes, enough lines in textfield for 2 paragraphs) ***** If they answer (B) to the first question....
