I need a widget to open a page where our fan base can input a song request for our band to learn. When posted it shall show... Full Name: Song Title: Artist: (optional) And a voting box on the displayed page in case someone viewing the requests likes a song already posted by someone else. I also would like the option to display the last 5,10, no limit requests on a side bar. And lastly, an email sent to the admin with full details like Full Name, Song Title, Artist, IP Address from who sent the song request.
