sourcing for encapsulation service

I am looking to have 2 pet pesticides put in capsules in different quantities and milligrams listed below and labeled, and safety banded in perscription bottles (see attached for how finished product will look) Keep in mind I need a lab or packager that can deal with small sized capsules...size 4 or 5 for most of the work below., Not all labs are set up for that size capsule...so you need to ask. They may also ask you for a COA of the powder which I can provide...and I also have a source in China for the actual powders themselves if they don't source it.
