Responsive HTML Form creation - With Clickable Motor Vehicle Diagram

HTML Form creation - Responsive Motor Vehicle Form Starts with only showing 3 fields, 2 input fields and 1 checkbox. Please see first screenshot Input field - Please holder text ‘Enter Reg Number’ Input field - Only numeric - Please holder text ‘Enter Mileage’ Chick box - Label - 'Average Mileage’ On entering these details, the form will expand showing all the need fields (When the user clicks 'HPI Check' it will auto populate all the fields). Please see second screenshot. The main part will be if there is an damage to register against the vehicle. It will either be a button or a tab which will present a clickable image of a motor car from above, where you can clearly click on the 2 front door wing mirrors, 4 wheels, 4 side doors, 1 boot door, front lights, rear lights and the front bonnet. Please see attached third image (Please image the checkboxes on the image as they are not needed) which is of a basic vehicle from above to give an idea but I would hope for the designer/development to be created and improve the style/quality. On clicking on any of these it would give a test area to save text against that clicked area labeled description, an input field for price and an option to upload a photo. For each element of the car clicked, it will build up a table with titles (Item / Price) i.e. Front Left Door - £99.99 Front Right Wheel - £ 120.00 Then a total of all the elements below. I am hoping to see and sign off the design with the agreed designer/developer, then be supplied html/css/javascript and I will finish the rest i.e. the PHP within my project.
