A form to create a Mecard

A form to create a me card that 1) generates a QR code pattern 2) writes the information from the form to a csv file The user of the form should be able to download the generated QR image to their computer. Ideally the size of the form should be 400px Fields on the form are: Title First Name Surname Street Name Town County Post Code Mobile Number Office Number Email Website url Notes In addition the form should ask these two questions Q1 Do you use business cards ?
