Device Magic Form builder format of inputs and defaults

We have created a form for a data gathering exercise using Device Magic. Our form consists of six sections/sub-forms and a total of around 50 fields. All we require at this stage is for someone to assist us with amending the form to ensure that the inputs are in the required form/in the correct format.
