Design some Stationery - "Save The Date"

I know exactly what I want but can't find it anywhere. I have a picture to send but basically its a mobile appt with month calendar showing September 2016 and the date "10" circled. A reminder will say: Save the Date, just below the calendar and then a few appointments will include the days events. It should look like a cell phone (iphone) with the cell bars on top left and other phone info. Most important, and what I can't get my phone to do is have the color filled appointments with the calendar on top. As I said I have a photo of what I'd like it to look like. I am also considering putting a picture of my kids as the phone case cover on the back (should also have phone camera lens). So it will be double sided but only that one button and an existing photo will be on the back. Looking for a couple day turnaround.
