Build a Conditional Form that Merges with Word Document

Looking to build a form that has several conditions (if statements). I am open to different options/platforms. It would be great to see what we can do with Microsoft Forms but we can use other pre-built form builders like Google forms. Otherwise, build an HTML form that can be hosted on website or as a simple desktop application (needs to work with both Windows and macOS). The responses from the form would then merge onto some type of Word or other template in order to automatically create a pdf that can be saved to the users desktop. Requirements include: 1. User must be able to merge responses with and create Word or Pdf document automatically on desktop or download from website (not have to send/receive doc via email) 2. Some questions would only appear if they answered a particular way to a question.... a specific answer may bring them straight to the end of the survey or bring up additional questions 2. Some response data would merge directly onto output document (name, contact info, etc.) 4. Remaining information would be filled in from default text (auto text or library of text) dependent on each response given... for example: - If response is "Yes" - insert text like: "Great job on your summary section!" - If response is "No" - insert text like: "We recommend building a strong summary section, here are some examples or resources etc. to help you build a comprehensive explanation of the professional you....." The default text could be a line or two or may be multiple paragraphs long. Approximate length is 15 to 20 questions or so.
