Check List ap for iOS and Adroid

To develop a simple 'Check List' Ap to be used on iOS and Android. - 5 Overall categories, with about 60 (total) items to "check" This check list will remind employees which 60 items are to be inspected before equipment is to be operated. If all items are inspected and approved, the check list is complete. If the investigation uncovers any items that are not Satisfactory, the Ap should ask the user if the item "Needs future attention", or "Needs immediate attention". The Ap will permit access to the mobile device's keyboard, voice recording and camera, to record a user's brief explanation of each item that is not Satisfactory. This information will then be sent to a supervisor via email. A notification to the supervisor via text message may also be warranted in some situations. INITIAL USE -Upon First use, Account information is entered: -- User name, phone number and up to 5 Supervisor names, email addresses and text phone numbers. (This 'Account' information should be accessible by the user whenever updates are required.) SUBSEQUENT USE Each 'Check List' use of the Ap thereafter: "Would you like to make an Inspection Report?" If yes, "SAFETY FIRST PLEASE MAKE SURE YOUR SAFETY VEST IS ON AND YOU ARE WORKING IN A SAFE AREA." - Inputs for user's Name and relevant equipment ID. (Auto populating from User's profile in Account Information, but allowing the user to correct and change when necessary.) *Time, Date and GPS location of each use are to be recorded automatically by the Ap. When all items in each category are "Checked", (or 'Satisfactory'), the next category appears. When any items are not "Checked" (i.e., not approved), then an information field is to appear at the end of each Category that will allow the user to classify each individual 'Non-Checked" item as "Needs future attention" or "Needs Immediate attention", and then allows the user to type, or voice record, and include photos, an explanation of the items that failed the inspection and will be reported to the supervisor. After all 5 categories are concluded,: -If all items are checked, "This concludes the check list" -If all items are either checked, or classified as "Needs future attention", then "This concludes the check list" -If any item is classified as "Needs immediate attention", then "Contact your supervisor immediately" should appear on the Ap. **In the end, each item on the list needs to be checked, or classified as "Needs future attention" or "Needs immediate attention". All collected information is then to be sent via email to the supervisor's emails via the email addresses listed in the initial user profile in Account Information. Anytime a "Needs immediate attention" is checked, a text is ALSO to be sent to the Supervisor's text numbers entered into the user's initial profile in Account Information; AND the completed results are to be sent via the email to the addresses as described above.
