Form has been created, it is a PDF form 2. I need the pdf form converted to HTML 3. I need to be able to fill out the form and hit submit 4. I need to be able to print form 5. I have the design for the form already made, so when you- remake my form it needs to be identical visually to the one thats already made. My budget is 15 Per form, if you can get this done quickly thats great. I was told that some one who is good with html could use the form as background put the fields over the phone do some html css work, and get this thing wrapped pretty quickly, theres a couple options to getting the work done. Im just looking for quality and turnaround time. Preferably today! if your ready to get started, and finish today great Freelancer preferred! Thanks Please see attached for Screen shot

I can start and finish tomorrow sir. It will look exactly as the two pictures. Some of the values are kinda hard to read particularly on the second form you will have to tell me what those values are at some point and also provide me with an email that you want all the information to be sent to and I can handle the rest. Around 2-3 hours of work I assume.
