Gather email addresses in existing php/mysql website

Currently, when someone searches for something, and advertisement shows up for 10 seconds, and then the results come up. Instead, there needs to now be an email sign up form on a 50% opaque background, which the data will be stored in a database. The admin area (which already exists) just needs to have a page linked to it that shows all of the email addresses, with checkmarks to each of them. At the bottom, there should be a 'download' button, that will download all of them in csv format that have the checkbox marked next to them. The signup form should use cookies and session variables. Cookies should be used to only show the form one time for each user. the session variabls should be used so it only shows up one time per visit. Ideally, if someone goes to the site to search, they should only have to sign up once. The users should only be able to get to their searches by filling out the form. The form will only be about 2 files - email and phone number. Please note also that the existing website is done in php/mysql, using object oriented programming. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law.
