On my site there are two pages where a user can download pdf documents. I want to create a page where the user needs to enter their name, email address and company name before they can download the file. These fields need to be validated before submission. Their name, email and company should be recorded in the database along with the name of the file that was downloaded.

A page will need to be created in the admin section which lists the files downloaded and the other information.
