5 minute job, tweaking an html web page.
1) I run a series of adwords campaigns which use a certain html page as a landing page. In the header there is a term that, depending on the ad that links to it, will be substituted with a different term.
2) There is a ten field form on the page, I'd like it so that each field is 'required' and the form won't be submittable until all fields are entered.

I run a series of adwords campaigns which use the page attached as a landing page. In the header of the page, notice the term "ACN". This term is also a keyword in one of the adwords ads I run that lands on this page. I'd like you to make it so that depending on the ad, a different term will be displayed instead of "ACN".
E.g. when I run an adwords ad with "Neways" as the keyword, I'd like "ACN" replaced by "Neways"
I'm running 18 different ads with the 18 corrosponding keywords below. I can put something in a string after the URL in each adwords ad if thats how you want to do it.

Also, there's a hidden field in the form at the bottom of the page that indicates to me, when I receive the form, which ad generated the form submission. As above, I'd like the value of the hidden field to be changed with every seperate ad that lands a visitor on the page. You'll find it above the "Submit" button in the code.
Finally, you'll notice in the form I have written 'marked fields are mandatory'. In fact, they aren't, and a visitor can submit the form without filling in the marked fields. I'd like you to make it so they are.
For example, if the user leaves out their email address, I want it so that the page reloads and something like "Email address is required" is displayed in the form under the line "marked fields are mandatory". Use any wording/font that would be appropriate.
