It could be made in EAGLE easily. How would you need these LEDs be populated. I guess it would have few series connection and some parallel connections of those series connected LEDs? Will deliver in a day. For the 12AWG wire connection, you could use a screw terminal if there is possibilities to have that or else it could be soldered directly into the board. 2 Layer board is OK for this job ? top llayer and bottom layer with PTH.
